Council are not able to provide records of immunisations given by doctors or other providers.
Most immunisation records, including those given by council are available by contacting the Australian Immunisation Register on 1800 653 809 or through your mygov account.
If you would like a copy of an immunisation record for immunisations administered by Buloke Shire Council's immunisation service, and that are not available on the Registry, please download the Immunisation Request Form and email it to immunisation@buloke.vic.gov.au
